Most procedures can be scheduled without a prior consultation visit to our office. Some procedures, however, require a consultation visit as your initial appointment and will consist of discussing your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation visit. Patients with a complex medical history or treatment plan will require an in-depth clinical evaluation first, and the surgical procedure will usually be scheduled for another day.
Please assist us by providing the following information at the time of your appointment:
If you have medical or dental insurance, bring the necessary completed forms with a copy of your insurance card(s). This will save time and allow us to help you process any claims.
Important: All patients under the age of 18 years of age must be accompanied by a parent or legal guardian during the appointment.
For patients undergoing IV sedation or general anesthesia, please do not eat or drink anything six (6) hours prior to your surgery appointment time. Also, you will need a responsible person to drive you home after your surgery.
Please alert the office if you have a medical condition that may be of concern prior to surgery (e.g., heart or lung disorders, stroke, cancer, diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.).
If your dentist or physician has taken x-rays, request that they forward them to our office. If there is not enough time for your x-rays to be sent, please pick them up and bring them to our office. If additional x-rays are necessary, we can take them at our office while you are here.
